Image-guided joint injections are minimally invasive procedures used to deliver medication directly into a joint with the assistance of imaging technology, such as ultrasound or fluoroscopy (real-time X-ray). These imaging techniques allow the healthcare provider to accurately position the needle within the joint, ensuring the medication reaches the intended area. Image-guided injections are commonly used to diagnose and treat joint pain caused by conditions such as osteoarthritis, inflammatory arthritis, bursitis, and sports injuries. Medications may include corticosteroids to reduce inflammation, local anesthetics for pain relief, or other therapeutic agents, depending on the condition being treated.

Patients taking blood-thinning medications may receive instructions about temporarily adjusting them if appropriate. On the day of the procedure, the skin over the injection site is cleaned thoroughly to reduce the risk of infection. In most cases, no general anesthesia is required, although a local anesthetic is commonly used to numb the skin and improve comfort during the injection.

How are Image-Guided Joint Injections Performed?

The procedure is typically completed in an outpatient setting and usually takes only a few minutes. The patient is positioned to allow easy access to the affected joint. After cleaning the skin with an antiseptic solution, a local anesthetic may be administered to numb the injection site.

The healthcare provider then uses ultrasound or fluoroscopic imaging to visualize the joint in real time and carefully guide a thin needle into the correct location. In some cases, a small amount of contrast dye may be injected during fluoroscopy to confirm accurate needle placement. Once proper positioning is verified, the prescribed medication is slowly injected into the joint space. The needle is removed, and a small bandage is applied. The imaging guidance helps maximize treatment effectiveness while minimizing the risk of inaccurate injection.

What to Expect During Recovery After Image-Guided Joint Injections?

Most patients return home shortly after the procedure and resume light daily activities the same day. Mild soreness at the injection site may occur for 24 to 48 hours and can usually be managed with ice and temporary activity modification. Pain relief may be immediate if a local anesthetic is used, while corticosteroids typically begin working within several days. Patients are often encouraged to gradually resume normal activities as symptoms improve. Follow-up appointments help evaluate the effectiveness of the injection and determine whether additional treatment is needed.

Although image-guided joint injections are generally safe, potential risks and complications include:
